About our Sliding Fee Scale Program
The Haven developed its Sliding Fee Program to offer affordable substance use treatment to individuals who are uninsured, under insured, or have low income. This program is available to all individuals who meet the income requirements, enabling them to receive care at a reduced cost. The Sliding Fee Program is available for our outpatient services, including the Intensive Outpatient Program.
Eligibility for the program is based on family size and annual household income. No one will be denied services based solely on inability to pay.
How do I qualify?
To qualify for the Sliding Fee Program, you must show proof of gross income for all household members. Gross income is all income before taxes. A household is defined as yourself (the applicant), your spouse, and children under 18 that live in your home.
How do I apply?
To apply for this program, please fill out and sign the application and return to The Haven via email or fax to 520-623-2065. When you submit your application, please include copies of your two latest pay stubs.
What is the sliding fee scale?
A calculation of the percentage of the Federal Poverty Guidelines is used in creating and annually updating the sliding fee schedule to determine eligibility. Please view the scale below.
